Normally it was supplied by the Head of the Institution.

It is the 13 th year of my service> I am yet to get any format from the head of my institution. If you can supply it please do it. We always write an application for the same.

I must supply it, but for this you have to wait up to 06/03/2013, because at present due to Administrator Nominee of Madhyamik Exam.-2013, I’m not attending my School right now.

West Bengal Form No. 2395 is meant for the purpose of taking Leave for W.B. S.G. Employees.
But, some Offices have customized CL, EL or Com. L. Application Formats just to save time of the employees and for easy tasking.
West Bengal Form No. 2395 is however attached.

it depends on head teacher whom he/she forwarded it…..few times to s.i of schools & some times to chairman also…….
