In case of OPD reimbursement is admissible for the following: (i) Malignant diseases, (ii) Tuberculosis, (iii) Hepatitis B/C and other liver diseases, (iv) Insulin-dependent diabetes, (v) Heart diseases, (vi) Neurological disorders/ Cerebrovascular disorders, (vii) Malignant malaria, (viii) Renal failure, (ix) Thallasaemia/ Bleeding orders/ Platelet disorders, (x) Injuries caused by accidents. Which category do you fall under? I think none. Then the answer is negative.

govt approved rate for this is rs.18000/-(code:01023022). it is a package rate like all other surgery. an empanelled n.h is bound to operate in this rate according to order.

i enrolled under WBHS2008 on 1st November 2011, but after that i diagnosed rarest Neurological Disorder( multiple sclerosis) which required to take Inj. Interferon one per week for indefinite time. the cost of medicine is 21000 (approx) per month. i am under the treatment of Bankura S Medical College & Hospital. i have to take under the supervision of physician. So i have to be admitted once in a month at BSMC&H and three other inj. will be taken at home(dr. has prescribed as follow up treatment for a month). i hope i will be reimbursed.plz guide me procedure.plz also inform me, is it a package or non package treatment in FORM D2?
