I have registered my name in the bank about three months ago. But I failed to take a print so I can not receive my user id and password. how do i get these nos.

How can I get the print out of the page of tempurary enrolment no of a new registered. Because after registration I didn’t take the print out of that existing page. I had tried farther many time but I could not get it. kindly help me as soon as possible then I verry thankfull to you .
