    In Govt. Offices Incumbency Registar is maintained for Gazetted posts, which lists name of Officers in chronological order displayed in the Office chamber.
    Now, if this register is lost what can the office do?

    make a new one with data available.

    If the data could not be recovered / recollected, then can it be prepared a fresh with existing data only?

    Yes, it may be done on the basis of available data in absence of old data.

    Many offices run without incumbency register because they have not sufficient information of the officers posted at that chair and their durations. There is no problem in it. And I think there is no audit objection if this register is missing.
