    Dear fellows,
    From a past experience in my Office, I would like to suggest you to fill up and submit the following forms (and modify them at the earliest, if felt necessary) which Govt. employees often delay, out of a casual approach that it can be done anytime.
    1. Nomination for General Provident Fund
    2. Nomination for General Insurance
    3. Nomination for Death Gratuity, and
    4. Declaration of Family.
    Forms are, I am sure, available in this forum.

    @Mr.Ray: Thanks for informing the Basics. In my office, as and when a new employee joins, I arrange to open service book, fill up relivant nomination forms(excepting GPF) and family declaration form. Of course all the forms are available in the link below.USEFUL FORMS IN EXCEL

    Dear Mr.Ray, the picture was not same when I first joined my Office. My senior said that opening of Service Book and the matters of Nomination will be delt with after completion of one years service. Being a novice, I had no other option but to agree with him. I am sure there are many Office till now where the basics are not followed properly. I think Govt. should do something for awareness over the fact.
