I am a state govt. employee. I want to make a passport. please guide me how to do.

http://passport.gov.in/cpv/dpPandvisaform.pdfThere is a standard format of the NOC which the intending Government employee requires to submit on receipt of the same from his appointing authority. The employee has to apply for the NOC.
N.B. – Online system was introduced, I am not sure whether whether it is continuing or not.Attached please find the latest G.O. regarding standard procedures that is to be followed for an ordinary post. There is also a NOC format attached with this G.O.
