NOTICE FOR ADMISSION TO 2 YEARS MULTI-SKILLED MALE HEALTH WORKERS DIPLOMA COURSE SESSION BEGINNING 1st NOVEMBER, 2014
I) Applications are invited by the State Medical Faculty of West Bengal for admission to MULTI-SKILLED MALE HEALTH WORKERS DIPLOMA COURSE: 2 Years + 6 months (Six months) stipendiary compulsory post examination training.
II) Centre for Teaching for the Course will be Netaji Subhas Sanatorium, Kalyani, Nadia. This is a Residential Course. Hostel facility is available at Netaji Subhas Sanatorium, Kalyani, Nadia.
III) Eligibility for Admission:
(a) Qualification: Passed Higher Secondary (10+2), with pass marks in Physics, Chemistry & Biology, individually, from any School in West Bengal, affiliated to a recognized University/Council/Board.
(b) That he has attained the age of 17 Years and upper age limit is 30 Years as on 03.11.2014
IV) Prospectus containing all details along with application form for the above Course can be obtained from the Office of State Medical Faculty of West Bengal at 14C, Beliaghata Main Road, Kolkata – 700 085 on all Working days (Monday To Friday) except Puja Holidays from 15.09.2014 to 13.10.2014 only between 10.00 A.M. to 5.00 P.M. on payment of Rs. 100/- by Cash/DD in favour of “State Medical Faculty of West Bengal” payable at Kolkata from 15.09.2014 to 13.10.2014.
However, candidate(s) can obtain Prospectus including application form by downloading website of the Faculty, www.smfwb.in and such filled in application along with DD for Rs. 100/- (Rupees One Hundred only) be submitted within due date at Faculty Office at 14C, Beliaghata Main Road, Kolkata – 700 085.
V) Last Date for submission of filled-in Application Form is 14.10.2014 (upto 5 P.M.) in the Office of the State Medical Faculty of West Bengal at the address mentioned above.
