The database of the SAM children released from NRC during 2013 & 2014 (till June) has been shared with the Food & Supplies Department and they are in the process of tagging the fair price shops (Ration) to each of the beneficiaries. A coupon booklet containing coupons for six months will be provided by the Food & Supplies Department and will be handed to the Chief Medical Officer of Health and DFWO, Kolkata.
In this regards the soft copy of the line list of the children discharged from the NRCs of your district for the above mentioned period (2013 & till June 2014) is being attached along with and you are requested to tag the ASHAs block wise with the beneficiaries so that the distribution of coupons can be made at an earlier date.

These coupons will be distributed to the beneficiaries in a onetime visit by the ASHAs/ HHW)/AWW/NGO partner. The beneficiaries will get their entitlement monthly from the fair price shop.
It is expected that the coupons will reach the CMOH’s /DFWO’s office on or before 29th September 2014.
You are requested to co-ordinate the matter so that the coupons reach the beneficiaries systematically.
