Are you a Govt. employee? then no worry. department will intimate you to open aadhaar card. if not, then no hurry. please wait for the local media to announce different enrollment camp in your city to assist opening aadhaar card. it is the responsibility of the govt. to provide every individual of India to give UID Aadhaar Card free of cost.

5: Yes, I personally know that it is still continuing at Central Bank of India near Kolkata Raj Bhavan at 4 & 4/1 Red Cross Place. Just visit the branch with required documents and get it done within an hour.

10: yes you can apply. PAN card is for identity proof and a residence proof as mentioned in this link.
