    Opening and maintenance of Service Book (appendix 7 of W.S.S.R.Pt-I)
    A Service Book in duplicate in prescribed form must be maintained from the date of appointment—one copy to be kept under custody of the Head Office—the other copy by the employee. In the case of W.B.C.S. (Ex.) & W.B.P.S. officers the Service records. will be maintained centrally by the Home (PAR) Deptt.
    Every step in the official life of the employee must be recorded in the S.B. with proper attestation and no erasure or overwriting is allowed; S.B. must be verified once in a year with the following certificate— Service verified upto (date) from ( the records from which the verification is made), if any portion of service cannot be verified from office records, a statement in writing by the employee as well as a record of evidence of his contemporaries should be attached to the Service Book.
    When an employee is transferred, upto-date entries must be made in the S.B. and forwarded to the new office. The S.B. in the custody of the employee must also be updated with proper attestation.
    The S.B. will be made in Bengali or in Nepali (for the Nepalese speaking employees) in addition to English.
    S.B. is to be shown every year to the employees under proper signature and a certificate to this effect is to be submitted by the officer to his higher authority.
    Verification of Service may be dispensed with by the Administrative Deptt. in terms of Memo. No. 2255F dt. 22.3.75 provided full particulars of service were already entered in the S.B. and sufficient reasons for non-authentication etc. are furnished.
    Personal certificates of character must not, unless the Head of Department so directs be entered in the S.B. but if an employee is reduced in rank, the reason of reduction must be noted. Results of disciplinary action/court case, if any should also be recorded.
    If an employee is transferred to foreign service, his S.B. must be sent to the Audit Officer concerned who will note necessary details therein (such as leave admissible or any other particulars) and record the same. On retransfer of the employee, the S.B. must again be sent to the Audit Officer for noting similar or other details as considered necessary.

    Dear sirs, Thanks for such healthy discussions. Today noon, some of my colleagues are trying to fill up the service book and then i told them to make a duplicate copy/book for future. after returning home, when i open this, it makes me happy. Thanks again. Our TIC don’t avail any facilities for that post, what he will write in his service book?

    Is there any GO regarding duplicate service book, because HM not ready to prepare and give the second copy. Please upload or give no., if any.
    Secondly, in service book, is it necessary to note – A. any ADI approved additional allowances such as conveyance allowance for disabled, B. MC resolution no. for permission given for higher degree such as PhD.

    1. There is no GO regarding duplicate Service Book, neither it is mandatory for an HM to prepare any such for any staff. But you can make a same copy in your own interest and get the signature of your HM on it for keeping it in your own custody, of course, if your HM is ready to sign on a second copy of Service Book.
    2. Yes, it is necessary to record –
    (a) the approval no. of any DI/ ADI approved additional allowances
    (b) MC resolution no. and also memo No. of permission letter from DI for permission given for higher degree such as PhD.

    Regarding higher degree-
    Permission was given by the MC on 2003, but no approval of ADI.
    Applied & informed to ADI, but he did not respond. I think in 2002-03 it was not mandatory, is it ?
    In this case should only the MC resolution for permission be noted ?
    Completed PhD on 2012. Should it be noted on first page column 4(b] ?
