Advertisement for Admission in Diploma Course in Ayurvedic Pharmacy for the session 2014 – 2015
Applications are invited for admission into two years’ Diploma Course in Ayurvedic Pharmacy in the Institute at Vishwanath Ayurved Mahavidyalaya & Hospital, Government of West Bengal, 94, Grey Street, Kolkata – 700 005.
A. Eligibility for Admission:
The students will be admitted to D.Pharm (Ayurveda) course, subject to fulfillment of the following conditions:
a) That he/she is a permanent resident of West Bengal and has passed class XII (10+2) Examination with Physics, Chemistry & Biology or Mathematics from a school recognized by the WBCHSE/ ICSE/ CBSE.
b) That he/she should not be less than 16 years and more than 30 years of age on 30th day of June of the year of admission. Relaxation of age for S.C & S.T. candidates is 3 years.
c) That he/she is of good character to be certified by the Head of the Institution where he/she studied last.
B. Submission of Application:
a) Application form will be available in the Departmental website i.e. www.wbhealth.gov.in.
b) Incomplete application shall be liable to be rejected.
C. Selection of Candidate:
a) Admission will be on merit basis. Merit list for admission will be prepared on the basis of marks obtained in Physics + Chemistry + Biology or Physics + Chemistry + Mathematics, whichever is higher, in the H.S. (10+2)/CBSE/ICSE. The merit list will be valid for 60 days only.
b) Counseling: Provisionally selected candidates for counseling will have to appear before the Admission Committee on the specified date and time when so called for, at their own expenses.
c) Total number of seats: 20 (twenty).
d) Reservation of Seats: As per existing Rules of Government of West Bengal.
Attested copies of the following documents are to be enclosed with application:
- Copy of the Mark Sheet of XII (10+2) Examination or equivalent.
- Copy of the Madhyamik Examination or Equivalent Examination Pass Certificate.
- Copy of the Age proof.
- Copy of the Character Certificate by the Head of last attended Institution.
- Copy of residential Certificate issued by the Competent Authority (Viz. M.P./ M.L.A/ Member of Zila Parishad/ Sabhapati of the Panchayat Samiti/ Commission of Municipality/ Councilor/ Prodhan of Gram Panchayat/ B.D.O of the area/ any Officer of or above the Rank of Asstt. Secretary to the Government of West Bengal, etc.) as per existing rules/ norms of Government of West Bengal.
- If belong to S.C./ S.T./ P.H. copy of the certificate issued by competent authority as per existing rules/ norms of Government of West Bengal.
Duly filled up applications as per proforma should reach the office of the “Superintendent & Ex-officio Professor, Vishwanath Ayurved Mahavidyalaya & Hospital, 94, Grey Street, Kolkata – 700 005,” by 26.06.2014 on or before 3.00 p.m. in a closed envelop, superscribing “Application for admission into Diploma Course in Ayurvedic Pharmacy for the session of 2014 – 2015“. Application should be sent either by Registered Post or Speed Post. Applications may also be submitted personally between 12-00 Noon to 3-00 P.M. on all working days except Saturday & holiday to the above address on or before the closing date.
This advertisement along with the proforma of application for admission will be available in the Official Website of the Department of Health & Family Welfare, Government of West Bengal (www.wbhealth.gov.in) and in the notice board of Directorate of Ayurveda (6th floor), Kolkata, and office of the Superintendent & Ex-officio Professor, Vishwanath Ayurved Mahavidyalaya & Hospital, 94, Grey Street, Kolkata -700 005.
The list of provisionally eligible candidates along with rules and regulations for admission will be available in the notice board, Swasthya Bhawan, Directorate of Ayurveda (6th floor), Kolkata, and Office of the Superintendent, Vishwanath Ayurved Mahavidyalaya & Hospital, 94, Grey Street, Kolkata – 700 005 and in the official Website of Department of Health & Family Welfare, Government of West Bengal (www.wbhealth.gov.in) on 16.07.2014. No separate communication will be made in this regard.
Applications reaching the office of the Superintendent, Vishwanath Ayurveda Mahavidyalaya & Hospital after 26.06.2014 will not be considered.
