COSA FORMAT for SCHOOL TEACHERS

The attached 4-page FORMAT is issued to all the school authorities.
All the staffs of an institution are asked to submit the same to the corresponding DI/ADI offices.
This is meant for a smooth implementation of COSA.I am a pass graduate non b.ed teacher.I have joined the A.T. post on 20/4/2006.I take My last increment on 1/7/2012 as 6th increment.Now my increment will be stop.Tell me when my next increment will be start or what should i write on no 10 column.(Next Increment date).I am Physics hons. graduate from C.U.But joined the assistant teacher post as pass graduate . should I mention Hons.in Physics in column 55 i.e educational qualification?

in col 55
you must write your all educational qualifications achieved so far.
in col 10
your increment, if stops now, will be automatically applied when you completes 10 year of service. so write the date accordingly.
you may please consult your HM and/or DI/ADI.
