Subject: Enquiry regarding Digitized Birth Certificates, Birth Certificate issued offline but not digitized for the period between 01.01.2021 to 31.12.2025.
Madam/ Sir,
It has been decided that all digitized/ delayed Birth Certificates issued during the period from 01.01.2021 to 31.12.2025 shall be verified to establish their genuineness.
In this regard, KMC Commissioner and District Magistrates are directed to ensure that the following actions are taken in a coordinated, time-bound and uniform manner:
1. Verification of Digitized Birth Certificates
The Block Development Officers (BDOs) in rural areas and the Sub-Divisional Officers (SDOs) in urban areas shall be responsible for conducting verification of all digitized/ delayed Birth Certificates issued during the aforesaid period. The BDOs/ SDOs shall constitute enquiry teams, wherever necessary, with special emphasis on cases involving delayed registration of Birth Certificates.
The enquiry shall include detailed examination of all relevant supporting documents, including manually issued Birth Certificates, relevant pages of the Birth Certificate Issuance Register, and any other associated records. All such documents, along with the findings of the verification/ enquiry, shall be uploaded to the Janma-Mrityu Tathya (JMT) Portal.
Each verification report shall clearly record the finding in respect of every individual Birth Certificate, specifically indicating whether the Birth Certificate is genuine, non-genuine, or irregular. The issuing authority’s competence and jurisdiction, as well as the authenticity and validity of supporting documents, shall be duly verified during the enquiry. In cases where any Birth Certificate is found to be fake, fraudulent, irregular, or unauthorized, the same shall be clearly identified, and detailed reasons for such conclusion shall be recorded.
2. Verification of Offline Birth Certificates
A similar verification exercise shall be conducted in respect of all offline Birth Certificates issued during the same period which have not been digitized. The concerned BDOs/ SDOs shall ensure examination of relevant registers and supporting records. The findings of the enquiry shall be recorded for each Birth Certificate individually, clearly indicating reasons in cases where the Birth Certificate is found to be non-genuine, irregular, or unauthorized.
The detailed SOP issued by Health & Family Welfare department regarding the procedure to be followed for conducting the verification/ enquiry and for recording findings by the Enquiry Officer shall be strictly adhered to ensure uniformity and accuracy. The entire exercise shall be given TOP PRIORITY and must be completed within the month of September.
